Dietitians at Home specializes in delivering medical nutrition therapy directly to the homes of geriatric patients. Our dedicated team of dietitians offers counseling and assistance to geriatric individuals managing diabetes and chronic kidney disease. Our services extend to patients in the broader Chicagoland area. We are actively seeking dietitians who are devoted to overseeing a group of patients, serving as enduring partners in their health journey. We are looking for Dietitians with the following qualifications:

Registered Dietitian Nutritionist (RDN) by the Commission on Dietetic Registration
Licensed Dietitian/Nutritionist (LDN) by the Illinois/Indiana Board of Dietetics and Nutrition

A successful candidate has the following qualities:

Confident and Independent
Interested in providing culturally competent nutritional therapy
Excellent listener / motivational interviewer
Highly organized and goal oriented
Interested in developing long term relationships with patients
Passionate about improving the outcomes of patients living with diabetes and chronic kidney disease
